Neighbourhoods
Designed for neighbourhoods, not just organisations

Primary care is no longer delivered by a single practice. It is delivered by neighbourhoods – groups of practices, teams, and services working together to support a shared population.

Engage Health Systems is built for that reality.

Our platform enables Primary Care Networks and neighbourhood teams to coordinate care, reduce variation, and deliver consistent, high-quality support across entire populations.

Care home support at neighbourhood scale

Care home residents are among the most vulnerable people in any neighbourhood and often the most affected by fragmented care.

The Engage care home module enables:

• Structured questionnaires guiding care home submissions
• Complete, consistent information captured at the start
• Clear triage: Urgent / Same Day / Routine
• A single, shared view of requests across the neighbourhood
• Improved visibility for PCNs and practices
• More streamlined workflows for all teams
• Better coordination and consistency in care home support

Who Do We Support?

Engage supports providers and patients with online access to community health and care services. It is available for all community services and PCNs whose constituent practices do not use Engage Consult as their primary online consultation system.

Engage has been designed to work as a stand-alone system or in conjunction with Engage Consult across a network of providers to manage patient care seamlessly. It is proven to improve data sharing between services, reduce inappropriate referrals and improve waiting times and outcomes for patients.

How can Engage Enhance Community Care?

It is the ideal online solution for:

  • Primary Care Networks managing care home activity, reducing emergency hospital admissions and readmissions.
  • IAPT Mental Health Services, gathering PHQ9 and GAD7 assessments pre-consultation to support triage and allocation to the most appropriate treatment option.
  • Urgent Care Providers, e.g., GP Walk-in centres, Urgent Treatment Centres and GP Out of Hours, providing call prioritisation and advance additional clinical information gathering pre-assessment.
  • Community Pharmacies, by supporting GP practices to manage medication requests, medication reviews and minor ailments.
  • Community Services such as:
    • Musculoskeletal (MSK)
    • Tele-dermatology
    • Sexual Health
  • Providing access to Advice and Guidance for professionals and patients and self-referral access to specialist community services. ICBs and ICSs providing local service provision can signpost patients to the right service, the first-time reducing demand in GP practices, secondary care, and emergency departments.
