Pharmacy Community

Across the UK, Community Care is delivered in a wide range of settings from patients own homes to community clinics, community centres, schools, and hubs.’

One of the many challenges for those working within community services is trying to coordinate patient care with other organisations, including non-NHS providers, whilst working closely with GPs, local hospitals, mental health trusts, pharmacies, and care homes.

We understand the complex issues faced by care providers, so we have developed a product specially for use with non-GP services within the community care setting.

Who Do We Support?

Engage Care Referral Service (CRS) supports providers and patients with online access to community health and care services. CRS is available for all community services and PCNs whose constituent practices do not use Engage Consult as their primary online consultation system.

Engage CRS has been designed to work as a stand-alone system or in conjunction with Engage Consult across a network of providers to manage patient care seamlessly. CRS is proven to improve data sharing between services, reduce inappropriate referrals and improve waiting times and outcomes for patients.

How can Engage Health’s Care Referral Service Transform Community Care?

CRS is the ideal online solution for:

  • Primary Care Networks managing care home activity, reducing emergency hospital admissions and readmissions.

  • IAPT Mental Health Services, gathering PHQ9 and GAD7 assessments pre-consultation to support triage and allocation to the most appropriate treatment option.

  • Urgent Care Providers, e.g., GP Walk-in centres, Urgent Treatment Centres and GP Out of Hours, providing call prioritisation and advance additional clinical information gathering pre-assessment.

  • Community Pharmacies, by supporting GP practices to manage medication requests, medication reviews and minor ailments.

  • Community Services. CRS is available for a wide range of community services such as:

    • Musculoskeletal (MSK)
    • Tele-dermatology
    • Sexual Health
  • Providing access to Advice and Guidance for professionals and patients and self-referral access to specialist community services. CCGs and ICSs providing local service provision can signpost patients to the right service, the first-time reducing demand in GP practices, secondary care, and emergency departments.

We support providers, CCGs, and ICS organisations by helping to improve organisational efficiencies, utilise community provided services and reduce specialist outpatient referrals. Engage CRS also improves data sharing between organisations, reducing the risk of complaints, mistakes, duplications, and polypharmacy.
